BackgroundOrgan donation has become one of the most effective ways to save lives and improve the quality of life for patients with end-stage organ failure. No previous studies have investigated the preferences for the different consenting options for organ donation in Egypt. This study aims to assess Egyptians’ preferences regarding consenting options for posthumous organ donation, and measure their awareness and acceptance of the Egyptian law articles regulating organ donation.MethodsA cross sectional study was conducted among 2743 participants over two years. (...) Each participant was required to rank eleven consenting options from 1 to 11, and to report his awareness and acceptance of the seven articles of the Egyptian law of organ donation.Results47% of the participants expressed willingness to donate their organs after death. This percentage increased to 78% when consenting options were explained to participants. “Informed consent by donor only” was the most preferred type of consent for one third of respondents. Awareness of the law articles regulating organ donation was relatively low ranging from 56% to 23%.ConclusionCurrently, around half of the Egyptian population agree to posthumous organ donation. This percentage could be increased significantly by raising the awareness about how the process of donation could be regulated and how the patient’s right of decision could be protected. (shrink)

In _Opposition to Philosophy in Safavid Iran_ Ata Anzali and S.M. Hadi Gerami offer a critical edition of what is arguably the most erudite and extensive critique of philosophy from the Safavid period. The editors’ extensive introduction offers an in-depth analysis that places the work within the broader framework of Safavid intellectual and social history.

The study aim was 2-fold (i) to describe and compare the in-season variations of acute: chronic workload ratio (ACWR) coupled, ACWR uncoupled, and exponentially weighted moving average (EWMA) through session-rated perceived exertion (s-RPE), total distance (TD), high-speed running distance (HSRD), and sprint distance across different periods of a professional soccer season (early, mid, and end-season) between starters and non-starters; (ii) to analyze the relationship the aforementioned measures across different periods of the season for starters and non-starters. Twenty elite soccer players (...) (mean ± SD age, 29.4 ± 4.4 y; height, 1.8 ± 0.1 m; and body mass, 74.8 ± 2.3 kg). They were divided into starter and non-starter groups and were evaluated for 20 weeks. ACWR had general changes throughout the season. At the beginning and end of the mid-season, the highest ACWR was observed in three parameters: s-RPE, TD, and HSRD. ACWR and EWMA through sprint distance were higher at the beginning of the early-season than at any other time of the season.The ACWR coupled of s-RPE shows a significant higher value for non-starters than starters (p = 0.015;g = −1.01 [−1.98, −0.09]) and the ACWR coupled of TD shows a significant higher value for starters than non-starters in early-season (p < 0.01;g = 3.01 [1.78, 4.46]) and shows a significant higher value for non-starters than starters in mid-season (p < 0.01;g = −2.52 [−3.83, −1.39]), and end-season (p < 0.01;g = −2.57 [−3.89, −1.43]). While the EWMA of TD shows a significant higher value for starters than non-starters in early-season (p < 0.01;g = 2.25 [1.17, 3.49]) and mid-season (p < 0.01;g = 2.42 [1.31, 3.71]), and shows a significant higher value for non-starters than starters in end-season (p < 0.01;g = −2.23 [−3.47, −1.16]). Additionally, we found some correlations between external and internal load measures during three periods of the in-season. The study’s main finding was that the indexes of ACWR and EWMA were useful to detect differences between period and between playing status with the exception for the sprint variable. In addition, the necessary work for non-starter players’ improvement is not done during training, and these players lose their readiness as the season progresses. Consequently, these players perform poorly during the match. Therefore, coaches and their staff should consider devising new activities to keep non-starter players physically fit. This deficit must be accounted for in training because they compete in fewer matches and have less burden than starters. (shrink)

We obtain in this paper a representation of the formulae of extensions ofL by generalized quantifiers through functors between categories of first-order structures and partial isomorphisms. The main tool in the proofs is the back-and-forth technique. As a corollary we obtain the Caicedo's version of Fraïssés theorem characterizing elementary equivalence for such languages. We also discuss informally some geometrical interpretations of our results.

Masjids, which are the center of collective worship in the religion of Islam, besides being a temple, it used to be school, hospital, public kitchen, guestroom, headquarters, court and meeting room and so on in the first centuries of hijri, and furthermore they became the center many religious, political and social functions. Prophet Mohammad's (peace be upon him) encouragements on the importance and virtue of performing prayers collectively enabled Muslims to pray together in mosques. There were children next to the (...) adults in the ranks kept in masjids and mosques and they also participated in the educational activities. When the hadith sources are researched from this point of view, some rumors (riwâyât) were encountered about keeping children away from the mosques. In the mentioned rumors, it is requested that not only children but also insane people, those who trade, those who speak in a loud voice, those who argue, those who draw swords at each other, are kept away from the mosques. Also according to this rumors, al-hudûd crimes shouldn’t be carried out in the mosque. Considering the size of the present study, it is limited to the rumors that include keeping children away from mosques. In this study, the takhrîj of the mentioned rumors were studied and they were criticized in terms of isnâd and matn. In the context of this method, if any, narrators who criticized in the integrity of isnâd have been determined, al-jarh al-ta‘dil status of the relevant narrators and the effect of this situation on the authentication of the rumor has been figured out in this study. While researching the rumors, the fundamental hadith sources are taken as a basis for this study. The ultimate goal of the study is to determine the authentication of the mentioned rumors in terms of both isnâd and matn. At the end of this study, it was concluded that the mentioned rumors were weak in terms of both isnâd and matn. (shrink)

Mathematics and logic are indispensable in science, yet how they are deployed and why they are so effective, especially in the natural sciences, is poorly understood. In this paper, I focus on the how by analysing Jean Piaget’s application of mathematics to the empirical content of psychological experiment; however, I do not lose sight of the application’s wider implications on the why. In a case study, I set out how Piaget drew on the stock of mathematical structures to model psychological (...) content, namely, the operations of thought involved in reasoning. In particular, I show how operations of thought form structured wholes that initially resisted modelling by either lattices or groups but could be modelled adequately by modifications of these mathematical structures. Piaget coined the term ‘grouping’ for the modified structure, and I conclude that it represents a non-canonical application of mathematics to the empirical content of experimental psychology. I also touch on the role external factors played in Piaget’s development of the grouping. According to the genetic epistemology conceived by Piaget, the origin of intelligence lies in the biological organism and develops in stages over time, and, via the grouping, Piaget established a genetic relationship between two stages of reasoning. I show how this relationship explains why mathematics and logic fit the psychological content of reasoning whilst simultaneously making their successful deployment in the natural sciences more mysterious. Finally, I turn to the explanation Piaget envisaged for the unreasonable effectiveness of mathematics in the natural sciences and consider some consequences for naturalism and Pythagoreanism. As part of a larger research study, we present model language for disclosing financial interests in clinical research to potential research participants, and we describe the empirical basis and theoretical assumptions used in developing the language. The empirical process for creating appropriate disclosure language resulted in a generic disclosure statement for cases in which no risk to participants’ welfare or the scientific integrity of the research is expected, and nine more specific disclosure statements for cases in which some risk is (...) expected. The disclosure statements are not meant to be canonical, but were instead designed to reflect the typical situations in which disclosure of financial interest might be considered by an institutional review board or conflict of interest committee. Individual institutions could modify key phrases to suit their purposes, and others could use the language in future empirical work on informed consent to better refine the options for disclosing financial interests in clinical research. Synaesthesia is a condition in which one type of stimulation evokes the sensation of another, as when the hearing of a sound produces photisms, i.e. mental percepts of colours. R is a 20 year old colour blind subject who, in addition to the relatively common grapheme-colour synaesthesia, presents a rarely reported cross modal perception in which a variety of visual stimuli elicit aura-like percepts of colour. In R, photisms seem to be closely related to the affective valence of stimuli and (...) typically bring out a consistent pattern of emotional responses. The present case study suggests that colours might be an intrinsic category of the human brain. We developed an empirical methodology that allowed us to study the subject's otherwise inaccessible phenomenological experience. First, we found that R shows a Stroop effect (delayed response due to interference) elicited by photisms despite the fact that he does not show a regular Stroop with real colours. Secondly, by manipulating the colour context we confirmed that colours can alter R's emotional evaluation of the stimuli. Furthermore, we demonstrated that R's auras may actually lead to a partially inverted emotional spectrum where certain stimuli bring out emotional reactions opposite to the normal ones. These findings can only be accounted for by considering R's subjective colour experience or qualia. Therefore the present paper defends the view that qualia are a useful scientific concept that can be approached and studied by experimental methods. (shrink)

DNA double‐strand breaks (DSBs) are extremely hazardous lesions for all DNA‐bearing organisms and the mechanisms of DSB repair are highly conserved. In the eukaryotic mitotic cell cycle, DSBs are often present following DNA replication while, in meiosis, hundreds of DSBs are generated as a prelude to the reshuffling of the maternally and paternally derived genomes. In both cases, the DSBs are repaired by a process called homologous recombinational repair (HRR), which utilises an intact DNA molecule as the repair template. Mitotic (...) and meiotic HRR are managed by ‘checkpoints’ that inhibit cell division until DSB repair is complete. Here we attempt to summarise the substantial recent progress in understanding the checkpoint management of HRR in mitosis (focussing mainly on mammals) and then go on to use this information as a framework for understanding the presumed checkpoint management of HRR in mammalian meiosis. Dual antiviral therapy with pegylated interferon-2a and ribavirin only reaches sustained virological response rates of ~30% after liver transplantation. With the approval of viral NS3/4A (...) protease inhibitors telaprevir, boceprevir, and simeprevir and the NS5B polymerase inhibitor sofosbuvir, combination therapy offers new therapeutic options for HCV-infected patients, resulting in considerably higher sustained virological response rates in the nontransplant setting. Case presentation: We report three cases of TVR-based triple antiviral therapy in HCV genotype 1 reinfected patients after liver transplantation, of whom a 57-year-old Caucasian female and a 43-year-old Caucasian male were therapy naïve, and a 49-year-old Caucasian male patient was pretreated ineffectively. After 4 weeks of therapy, viral load decreased one to three log10 and became negative in weeks 6 to 8 in the therapy naïve patients. The pretreated patient showed a negative viral load in week 4. TVR was administered over 12 weeks, 750 mg thrice daily. Doses of immunosuppression with cyclosporine were reduced four to six fold. Initial peg-IFN and RBV doses ranged from 135–180 µg/week and 800–1,200 mg/day, according to the patient's body weight. Doses of peg-IFN and RBV were adapted to 90–135 µg/week and 400–800 mg/day after 2 to 12 weeks of protease inhibitor therapy. Dual therapy was continued for 36 weeks with total treatment duration of 48 weeks in the therapy naïve patients leading to a sustained virological response 12 weeks after the end of therapy. In the pretreated patient a breakthrough was detected in week 24 and therapy was discontinued. Overall, antiviral therapy was well tolerated. Side effects included dysgeusia and anemia leading to erythropoietin application and blood transfusions. Conclusion: This case series emphasizes that triple therapy with TVR is an efficient treatment for therapy naïve HCV genotype 1 reinfected patients after liver transplantation. The objective of this work is to categorize the available fly ashes in different parts of the world into distinct groups based on its compositional attributes. Kohonen's self-organizing feature map and radial basis function networks are applied in a cascading fashion for the classification of fly ashes in terms of its chemical parameters. The basic procedure of the methodology consists of three stages: apply self-organizing neural net to ascertain possible number of groups, delineate them and identify the group sensitive attributes; (...) find mean values of sensitive attributes of the elicited groups and augment them as start-up prototypes in k-means algorithm and find the refined centroids of these groups; incorporate the centroids in a two layer radial basis function network and fine-tune the delineated groups and develop an indexing equation using the weights of the stabilized network. Further, to demonstrate the utility of this classification scheme, the so formed groups were correlated with their performance in High Volume Fly Ash Concrete System [HVFAC]. The categorization was found to be excellent and compares well with Canadian Standard Association's [CSA A 3000] classification scheme. (shrink)

In his paper titled ‘Against “measurement” ’ [Physics World 3(8), 33–40 [1990]], Bell criticised arguments that use the concept of measurement to justify the statistical interpretation of quantum theory. Among these was the text of Gottfried [Quantum Mechanics (Benjamin, New York, [1966])]. Gottfried has replied to this criticism, claiming to show that, for systems with both continuous and discrete degrees of freedom, the statistical interpretation for the discrete variables is implied by requiring that the continuous variables are described classically. In (...) the present paper, Gottfried’s argument is criticised. It is suggested that he takes over aspects of classical physics which are in conflict with the classical limit of the Schrödinger equation. He incorrectly assumes that, in the output from a Stern-Gerlach apparatus, the wave-function of any ion is restricted to one or another of the beams. (shrink)

In our commentary we ask whether we should ultimately endeavour to find the deep causes of behaviours? Then we discuss two extensions of the proposed framework: (1) Mendelian randomisation and (2) hypothesis-free gene–environment interaction (leveraging heterogeneity in genetic associations). These complementary methods help move us towards second-generation causal knowledge, ultimately understanding mechanistic pathways and identifying more effective intervention targets.

This note comments on a recent paper by Reingold, itself a response to remarks by Gillispie at the 1981 meeting of the American Association for the Advancement of Science.

O, bu dönemde düşünce akımlarının etkisiyle gelişen inanç problemleri karşısında geleneksel yöntem ve bilgi birikiminin etkinliğini gündeme getirmiştir. Mustafa Sabri, İslam’ın temel kaynaklarından biri olan sünnet ve bunun kayıt altına alındığı rivayetleri içeren muteber (...) hadis kaynaklarının metot ve sıhhat açısından son derece güvenilir olduğunu ve itikadî konularda belirleyici olduğunu vurgulamıştır. Mustafa Sabri’nin sünnet tasavvurunu göstermesi açısından onun nüzûl-i Îsâ konusundaki görüşleri önemli bir örnektir. O, nüzûl-i Îsâ hadisesi hakkındaki yaklaşımını, Mahmut Şeltut’a yönelik itirazlarıyla ortaya koymaktadır. O, bu hadisenin Hz. Peygamber’in gayba ilişkin verdiği haberlerden biri olması ve kıyamet alametlerinden sayılması noktasında, sünnetin kaynaklığını öne çıkarmaktadır. Nüzûl-i Îsâ’nın gerçekleşme imkânını günümüzde sıhhati açısından problemli kabul edilen rivayetler, sahabe, tabiûn ve sonraki İslam âlimlerinin ortaya koyduğu yaklaşım ve hadis kaynaklarına duyduğu itimat üzerinden inşa etmektedir. Mustafa Sabri bu anlamda geleneksel birikimi öne çıkaran bir sünnet tasavvuru ortaya koymaktadır.
